Acupuncture for chronic shoulder pain in persons with spinal cord injury: a small-scale clinical trial
OBJECTIVE: To determine the efficacy of acupuncture in the treatment of chronic musculoskeletal shoulder pain in subjects with spinal cord injury (SCI). DESIGN: Randomized, double blind (participants, evaluator), placebo (invasive sham) controlled trial. SETTING: Clinical research center. PARTICIPANTS: Seventeen manual wheelchair-using subjects with chronic SCI and chronic musculoskeletal shoulder pain. INTERVENTIONS: Participants were randomly assigned to receive 10 treatments of either acupuncture or http://www.easymedonline.com/Mobility-Scooters-s/20.htm, invasive sham acupuncture (light needling of nonacupuncture points). MAIN OUTCOME MEASURE: Changes in shoulder pain intensity were measured using the power wheelchairs User’s Shoulder Pain Index. RESULTS: Shoulder pain decreased significantly over time in both the acupuncture and the sham acupuncture groups (P=.005), with decreases of 66% and 43%, respectively. There was no significant difference between the 2 groups (P=.364). There was, however, a medium effect size associated with the acupuncture treatment. CONCLUSIONS: There appears to be an analgesic effect or a powerful placebo effect associated with both acupuncture and sham acupuncture. There was a medium treatment effect associated with the acupuncture, which suggests that it may be superior to sham acupuncture. This observation, along with the limited power, indicates that a larger, more definitive randomized controlled trial using a similar design is warranted

Use of three gait-training strategies in an individual with multiple, chronic strokes

BACKGROUND AND PURPOSE: There is little information available regarding gait-training interventions for people with chronic, multiple strokes who are nonambulatory. The purpose of this case report is to describe the feasibility and power wheelchairs, www.easymedonline.com outcome of three different task-oriented gait-training techniques in an individual with chronic, multiple strokes who was not able to ambulate independently. CASE: The participant was a 61-year-old man with chronic quadriparesis resulting from five brainstem strokes sustained over five years previously. He maintained home and community mobility with a power wheelchair but sought to ambulate home and short community distances without assistance. Three six-week gait-training interventions were implemented sequentially twice per week: (1) overground gait training with manual assistance, (2) body weight-supported treadmill training (BWSTT), and (3) overground gait training with variable task practice. OUTCOME: Independent home and short community ambulation was not achieved by this individual after these interventions. However, he improved gait speed and endurance as well as balance over the 18-week intervention period. DISCUSSION: There appeared to be no advantage to BWSTT over traditional gait training for this individual, as improvement was noted during all three interventions at a comparable rate. Although these gait-training interventions facilitated gains in gait and balance measures during the treatment, no change in functional ambulation status was achieved. Given the time, financial, and labor demands involved with BWSTT and the lack of apparent benefit, traditional overground therapeutic intervention was reasonable and appropriate for this participant with a history of chronic, multiple strokes.

Reliability and validity of the power-mobility community driving assessment.

This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices

The truth of the matter is that there are more people who fail at their money making

These people are not told that they will actually have to work twice as hard at their <a

understand conversion rates or the concept of PPC (pay per click) programs.
However, they truly believe what they have been told - They believe that they can get rich by

doing absolutely nothing. These are otherwise intelligent people. Why would they believe this?

They believe because they want to believe. Because they see it everyday, everywhere on the

Internet. They are told that it is possible. They not only want to believe it - for their own

reasons, they need to believe it.
Unfortunately, it is my job to tell them the truth and then they feel let down. By the time a

client comes to me for help, they are already a bit disillusioned. Since initial clients

usually begin by paying for one session at a time, I go ahead and burst their bubble during the

first session. I don’t want them to spend money that they don’t have on an idea that won’t

work.
After the first session, the client is left to make a decision. They either have to do what it

takes to make their business a success, toss the idea out the window, or keep doing exactly

what they have been doing with the exact same results (or lack of results as the case may be).

Less than half are willing to do what it takes to succeed. The rest toss the idea out the

window. I’ve never had a client who decided to keep doing what they had been doing.
I do have a point. My point is that if you are an affiliate manager, or if you have a downline,

be honest. Tell your affiliates and downlines the truth. Tell them upfront what will be

required of them. Be honest about possible earnings, and be honest about your own earnings.

After being honest, “look around” to see who is still interested in your business. I promise,

there will still be people interested - people willing to put forth the effort that is

required. These are the people that you want to invest your time in.
Once you know who is serious about the business, work with them closely. Offer them all of the

support you can possibly offer. Make suggestions to help them improve their site. Put together

marketing packages and ideas for them. Start a forum for your downline, and be active in that

forum. If you haven’t heard from them in a while, send them an email or a card and ask them if

everything is okay. The more successful your downline is, the more successful you are.

The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

Twenty-five studies that met the inclusion criteria were reviewed. Nine studies reported the frequency of asphyxial deaths caused by physical restraints, nine studies reported the long-term complication and indirect adverse effects of physical restraints and lap-belt use, and seven studies reported the benefits of physical restraints and lap belts with individuals using medicare power wheelchairs, handicap scooters. Despite the weak evidence, the results suggest a considerable number of deaths from asphyxia caused by the use of physical restraints occurred each year in the U.S. The majority of the deaths occurred in nursing homes, followed by hospitals, and then the home of the person. Most deaths occurred while persons were restrained in wheelchairs or beds. Based on that, caution needs to be exercised when using restraints or positioning belts.
